Information we collect

When you use our request form, we collect the details you choose to give us: your name, email address, phone number, ZIP code, organization (for providers), the service you are interested in, how and when you prefer to be contacted, and your message. We ask you not to include medical details in the form.

How we use it

We use this information only to respond to your request, answer your questions, confirm whether we can serve you, and schedule visits or consultations. We do not sell your information and we do not use it for advertising.

How it is stored and shared

Form submissions are stored securely with the service provider that hosts this website and are shared only with the Argenti Health team members who respond to requests. We keep submissions for up to 12 months unless they become part of your care. To limit abuse, our systems briefly keep a one-way scrambled version of the network address a request came from.

Cookies and third parties

This website does not use advertising or analytics cookies. Pages load fonts from Google Fonts, which receives your device's network address when the fonts are downloaded.

Health information

Health information we collect while caring for you as a patient is handled separately, in accordance with applicable health privacy laws, including HIPAA.

Your choices

You can ask us to update or delete the information you sent through this website by emailing [email protected].

Changes to this notice

If we change this notice, we will post the new version on this page with a new effective date.
